ACCEPTING BRAVO GREATER DKS MOINES FY15 CAPITAL CAMPAIGN AND
PROJECTS GRANT AGRKKMKNT [RACCOON RIVER VALLEY TRAILHKAD
PUBLIC ART PRO JKCT]
IN THE NAME AND BY THE AUTHOMTY OF THE CITY OF WA UICEE,IOSM
WHEREAS,the City of Waukee,Dallas County,State of Iowa,is a duly organized
Municipal Organization;AND,
WHEREAS,on October 20,2014,the Waukee City Council authorized the submittal of a
grant application to the Bravo Capital Campaign Grant Program on behalf of the City of
Waukee and the Waukee Trailhead Public Art Project {Resolution 014-283),AND,
%HERKAS,the Bravo Capital Campaign Corrnnittee has awarded $25,000.00 to the City of
Waukee for the Waukee Trailhead Public Art Project with the provision that the money will
be spent within one (1)year of receipt and in the manner indicated in the grant application.
NOW THEREFORE BE IT RESOLVED by the City of Waukee City Council that it
hereby accepts the terms of the Bravo Greater Des Moines FY15 Capital Campaign and
Projects Grant Agreement [Raccoon River Valley Trailhead Public Art Project];
BK IT FURTHER RESOLVED that the Mayor of the City of Waukee is authorized to sign
the Bravo Greater Des Moines FY15 Capital Campaign and Projects Grant Agreement,which
is attached hereto as Exlubit A,on behalf of the City of Waukee.
Passed by the City Council of the City of Waukee,Iowa,and approved the ""day of
February,2015

TERMS OF GRANT
TAx ExEMPT STATUs:The grantee hereby represents and warrants that it is exempt from taxation by the
United States Government pursuant to Internal Revenue Code section 501(c)(3)or is an agency of a
local government that maintains a 28E agreement with Bravo Greater Des Moines.
I [.EXPENDITURE OF GRANT FUNDS:
ExPENDITURE QF REBPQNBIBILITY:The grantee represents and warrants that it intends to use the
funds provided by Bravo Greater Des Moines within one year of receipt and in the manner
indicated in its original application or as varied herein and that such use will be in accordance with
the tax-exempt activities for which a determination of exemption from taxes was received.
REVERsloN:AII funds not expended for the purposes agreed to by the grantee must be returned to
Bravo Greater Des Moines.
EXPENDITURE REPORTING:The grantee agrees to provide Bravo Greater Des Moines withafinalreportingofallexpendituresviatheBravowebsitebyJanuary15,2016.
Pusuc ANNDUNcEMENTS:Announcement of the grant may be made on a voluntary basis by the grantee
unless otherwise indicated by Bravo Greater Des Moines.Opportunities encouraged include:a.Distribute a press release announcing grant.
b.Acknowledge Bravo Greater Des Moines and partner.local governments in all complete listings of
donors (printed materials,website,etc.)whenever possible.
c.Include the Bravo Greater Des Moines logo whenever possible.
d.Include a link to the Bravo Greater Des Moines website on your organization's website if possible.e.Promote the Bravo Greater Des Moines Annual Awards Gala whenever possible.
Please send copies of any articles/publications relative to Bravo support to Bravo Greater Des Moines.
Please sign both copies and return one copy of this agreement by February 12,2015 to Bravo Greater Des Moines,2600 Grand Ave.,Suite 120,Des Moines,Iowa 50312.
The Capital Campaign 8 Projects final/progress report will be provided to Bravo Greater Des Moines on or
Authorized Grantee
