APPROVING ADDENDUM NO.1 FOR THE WAUIMK PUBLIC LIBRARY HVAC
IMPROVEMENT PRO JKCT (COMMISSIONING WORK)IN THK AMOUNT
OF $2,400.00
IN THE NAME AND BY THE A UTHORITY OF THE CITY OF WA UKEE,IOWA
WHEREAS,the City of Waukee,Dallas County,State of Iowa,is a duly organized Municipal
Organization;AND,
WHEREAS,on February 2,2015,the Waukee City Council approved retaining the services of
SystemWorks,LLC,of West Des Moines,IA,for the connnissioning portion of the Wmikee
Public Library HVAC Improvement Project (Resolution 815-039);AND,
WHEREAS,based on the improvements completed to date for thc project,SystemWorks,LLC,
has submitted Addendum No.1 to the commissioning work contract,attached hereto as Exlnbit
A,in order to conduct thermal imaging of the Waukee Public Library to identify areas of
excessive air infiltration;AND,
WHEREAS,City staff and the project engineer recommend approval of Addendum No.1.
NOW THEREFORE BE IT RESOLVED by the City Council of the City of Waiikee,Iowa on
tins 9"'day of February,2015,that Addendum No.1 for the Waukee Public Library HVAC
linprovement Project (Connnissioning Work)in the amount of $2,400.00 is hereby accepted and
approved.

SystemWorks will provide additional labor at rate of $120.00/Hour per person plus material and expenses
for work that is requested beyond the "Scope of Work"proposed.
SystemWorl&s will not perform additional work without prior approval of the owner.

Thermal Imaging
~A minimum ten degree delta between inside and outside temp is needed.
e Lltilize Ti32 Fluke thermal imaging camera to view and analyze the building envelope.This will
include roof,exterior walls and windows.
Destructive testing is not included.
~Final Thermal imaging Report Generation.
o Electronic copy will be provided.
o Pictures (IR+Actual)of concerns with details and notes of each item will be included,
Final report review /lessons learned meeting with MODUS /owner.
Clarifi casions:
~SystemWorks has not included time to return to job site and re-test after corrections have been
made,Time used for this service would be considered additional services.
SystemWorks has not included time for other trades (carpenter,roof,masonry,etc.3
~SystemWorks is not responsible for undiscovered deficiencies,
