RESOLUTION 16-384
APPROVING MIDAMERICAN ENERGY COMPANY AGREEMENT
[NE DOUGLAS PARKWAY FROM NE ALICE'S ROAD TO NE BADGER LANK]
IN THE NAME AND BY THE A UTHORITY OF THE CITY OF 8'A UICEE,IOWA
WHEREAS,the City of Waukee,Dallas County,State of Iowa,is a duly organized Municipal
Organization;AND,
WHEREAS,on March 14,2016,the Waukee City Council approved plans,specifications,form
of contract and estimate of cost (Resolution 416-084)for the NE Douglas Parkway Extension
Project;AND,
WHEREAS,MidAmerican Energy proposes an agreement,attached hereto as Exhibit A,to
install street lights on NE Douglas Parkway from NE Alice's Road to NE Badger Lane;AND,
WHEREAS,City staff recommends approval of the agreement.
NOW THEREFORE BE IT RESOLVED by the City of Waukee City Council in session this
6'"day of September,2016,that it hereby approves the MidAmerican Energy Company
agreement [NE Douglas Parkway from NE Alice's Road to NE Badger Lane].

There will be a one-time installation charge I'or labor and materials in the amount of $(42,330.19.Since this project
is over $100,000 you are required by tariff to pay actual costs upon completion of the work.If the project is less
than the original estimate you will be refunded the difference and conversely you will be charged any amount above
the estimated cost stated in this agreement.
MlidAmencan Energy Company's street light proposal is based on the approved street light analysis provided by the
City.MidAmerican Energy Company will install the lights based on the staked locations provided in the field by
the customer.
If this proposal meets with your approvaL please sign and return one (1)copy of this letter and one (I)copy of the
print.The other copy is for your file.
Upon receipt of your approval,we will prepare work instructions and schedule the installation of these lights.This
proposal is valid for 90 days and if lii[idAmerican Energy Company constniction has not commenced within 12
months it may be voided.
