AGENDA ITEM:Consideration of approval of Change Order No. 5 to Elder Corporation
for the Waukee Public Library Stormwater Improvements increasing the
contract amount by $8,114.40.
FORMAT:Consent Agenda
SYNOPSIS INCLUDING PRO & CON: Change Order No. 5 is for erosion control matting
placed on the steeper slopes around the pond and along the south
entrance.
FISCAL IMPACT INCLUDING COST/BENEFIT ANALYSIS: The net result of this
Change Order increases the contract price in the amount of $8,114.40.

Mr. Koester,
Please find attached Change Order #5 for the Waukee Public Library Stormwater Improvements. The
change order includes the following items:
· Erosion Control Matting
The erosion control matting is to be placed on steep slopes around the perimeter of the pond and on the
southern slope of the property. The matting will help protect the seeding that has been installed in
these areas. The matting will also help to prevent erosion caused by stormwater.
We have reviewed the change order items and unit prices, both are line with anticipated amounts. If
City staff concurs with this change, we recommend the City of Waukee approve this change order in the
amount of $8,114.40 to the construction contract for the Waukee Public Library Stormwater
Improvements with Elder Corporation of Pleasant Hill, IA.

THE CONTRACT IS CHANGED AS FOLLOWS:
Change Order Item CO-5.1 is to add an erosion control blanket to the project. The erosion control blanket will be placed in areas with steep slopes, specifically
around the pond and the slope on the south side of the driveway. The ersoin control blanket will help reduce the erosion of these slopes and aid in the
establishment of the seeding of these areas.
