FORMAT:Consent Agenda
SYNOPSIS INCLUDING PRO & CON: Change Order No. 1 is for additional clearing and
grubbing resulting of 3 trees in conflict with proposed improvements.
FISCAL IMPACT INCLUDING COST/BENEFIT ANALYSIS: The net result of this
Change Order increases the contract price in the amount of $9,900.00.

Warrior Lane Improvements - Phase 1
Dear Mr. Koester:
Please find attached Change Order #1 for the Warrior Lane Improvements - Phase 1 project. The
change order includes the following items:
• Additional Clearing & Grubbing
The additional tree removal will take place along Northview Drive west of Warrior Lane where two trees
are close to or in conflict with proposed improvements. In addition, a third tree in the same area will be
removed due to utility excavation near the base that could potentially compromise its future health and
stability. This will result in reduced future maintenance needs.
We have reviewed the change order items and unit prices, and both are in line with anticipated
amounts. If City staff concurs with this change, we recommend the City of Waukee approve this change
order in the amount of $9,900.00 to the construction contract for the Warrior Lane Improvements –
Phase 1 with Concrete Technologies, Inc. of Grimes, IA.

THE CONTRACT IS CHANGED AS FOLLOWS:
Change Order Item CO-1.1 is to add additional tree clearing and grubbing to the project. The work includes clearing and grubbing for three trees between Northview Drive and
Ashworth Drive, west of Warrior Lane. Two trees along Northview Drive were not identified for removal in the original project plans, but need to be removed due to close proximity
to or conflict with proposed improvements. A third tree in the same vicinity was originally marked to remain, but will now be removed due to utility excavation near the base that
could potentially compromise its future health and stability.
