AGENDA ITEM:Consideration of approval of Change Order No. 1 to Northway Well and
Pump Company for the 2021 Well Rehabilitation Project increasing the
contract amount by $9,734.50.
FORMAT:Motion
SYNOPSIS INCLUDING PRO & CON: Change Order No. 1 for a casing pipe size change
and to adjust the completion date for the project due to the necessary
material change.
FISCAL IMPACT INCLUDING COST/BENEFIT ANALYSIS: The net result of this
Change Order increases the contract price in the amount of $9,734.50.

2021 WELL REHABILITATION
CHANGE ORDER NO. 1
PAY ESTIMATE NO. 1
Enclosed are three signed copies of Change Order No. 1 and Pay Estimate No. 1 in accordance
with the contract between the City of Waukee and Northway Well and Pump Company for the
2021 Well Rehabilitation project.
Change Order No. 1 is for a casing pipe size change. Pay Estimate No. 1 is for mobilization and
casing pipe installation.
Change Order No. 1 will increase the price for the project by a total of $9,734.50. We
recommend approval of Change Order No. 1 by the City Council of the City of Waukee. We
have checked the pay estimate and recommend payment by the City Council of the City of
Waukee to Northway Well and Pump Company in the amount of $244,245.48.
Please execute all three copies of Change Order No. 1 and Pay Estimate No. 1 in the spaces
provided. Please return one signed copy to Veenstra & Kimm, Inc., forward one copy to
Northway Well and Pump Company and retain one copy for your files.
