Maintenance for the Warrior Lane Joint Utility Trench project increasing
the contract price in the amount of $24,000.00
FORMAT:Motion
SYNOPSIS INCLUDING PRO & CON: Change Order No. 1 is for reduction in the quantity
of Type I handholes, increase in the quantity of Type IV handholes and
reduction in the quantity of handholes provided by Utility Providers.
FISCAL IMPACT INCLUDING COST/BENEFIT ANALYSIS: The net result of this
Change Order increases the contract price in the amount of $24,000.00.

Mr. Koester,
Please find attached Change Order #1 for the Warrior Lane Joint Utility Trench. The change order
includes the following items:
• Reduction in the quantity of Type I Handholes
• Increase in the quantity of Type IV Handholes
• Reduction in the quantity of handholes provided by Utility Providers
This change order eliminates the Type I Handholes on the project and changes all the handholes to
Type IV. Type I handholes are small round handholes and Type IV handholes are larger rectangular
handholes. After bidding the unit prices for the Type I and Type IV were the same and through
discussions with City Staff, the contractor, and the engineer it was decided to recommend changing the
handholes to the larger type IV. Additionally, the unit price of $2,600 for type IV handholes is a
favorable price in regard to historical perspectives and thus additional type IV handholes are included in
this change order to add value to the Joint Utility Trench.
We have reviewed the change order items and unit prices, both are line with anticipated amounts. If
City staff concurs with this change, we recommend the City of Waukee approve this change order in the
amount of $24,000 to the construction contract for the Warrior Lane Joint Utility Trench with TD&I Cable
Maintenance of Lakeland, MN.

THE CONTRACT IS CHANGED AS FOLLOWS:
CO-1.1 - Deduction of qty 20 Type I Handholes, Furnish and Install. These will be replaced with Type IV Handholes.
CO-1.2 - Addition of qty 30 Type IV Handholes, Furnish and Install. These replace the 20 Type I Handholes and add an additional 10 Type IV handholes to provide
a consitancy between the 4 Handholes Banks (10 Handholes at each bank)
CO-1.3 - Deduction of qty 1 Handhole and Junction Box, Furnish and Install Only. It was discussed with the utilities, and no utilities requested to have the
contractor install an utility provided handhole.
