FORMAT:Consent Agenda SYNOPSIS INCLUDING PRO & CON: The Request for Additional Services is related to reimbursable expenses for permit fees and the printing and distribution of bidding document and on-line hosting through Action Reprographics ($2,200), completing custom design restroom bid documents ($3,300), and construction staking for the duration of the project ($7,500). [WESTOWN MEADOWS PARK IMPROVEMENT PROJECT] IN THE NAME AND BY THE AUTHORITY OF THE CITY OF WAUKEE, IOWA WHEREAS, the City of Waukee, Dallas County, State of Iowa, is a duly organized Municipal Organization; AND, WHEREAS, on October 15, 2018, the Waukee City Council approved an Agreement for Professional Consulting Services with Shive-Hattery, Inc. [Westown Meadows Park Implementation] (Resolution #18-442); AND, WHEREAS, Shive-Hattery, Inc., has submitted Amendment No. 1 to the agreement for reimbursable expenses for permit fees and the printing and distribution of bidding document and on-line hosting, completing custom design restroom bid documents, and construction staking for the duration of the project. NOW THEREFORE BE IT RESOLVED by the City Council of the City of Waukee that the Professional Services Agreement Amendment No. 1 with Shive-Hattery, Inc. [Westown Meadows Park Improvement Project], attached hereto as Exhibit A, is hereby approved. Lyon Project #418110-0 PROFESSIONAL SERVICES AMENDMENT TO: City of Waukee, Iowa ATTN: Matt Jermier Waukee, Iowa 50263 PROJECT: Westown Meadows Park Implementation ORIGINAL AGREEMENT DATE: October 15, 2018 AMENDMENT DATE: January 31, 2019 City of Waukee, IA and Shive-Hattery, Inc. (S-H) agree to amend the Original Agreement as follows: SCOPE OF SERVICES: Shive-Hattery, Inc. (S-H) will provide the following additional services: 1. Complete custom designed restroom bid documents including: a. Detailed drawings b. Technical specifications c. Effort required: $3,300 2. Reimbursable expenses for permit fees and the printing and distribution of bidding documents and on-line hosting through Action Reprographics: a. This amendment reflects direct costs of up to $2,200. 3. Construction Staking: A. MOBILIZATION / CONTROL NETWORK 1. This service is an integral part of this proposal and must be performed prior to executing any other services or additional services 2. Establish horizontal and vertical control for layout and construction of improvements as detailed in plans. These control points remain the property of Shive-Hattery and any restaking of such will be additional services attributable to the contractor. The following services will be performed following authorization by client. Each item would be performed once under this contract in accordance to the above referenced project. B. REMOVALS: 1. Provide staking necessary to mark limits for grading and sawcut lines for removals as defined by plans. 2. Provide marks painted on surface to mark sawcut lines for removals as defined by plans. C. STORM AND SANITARY SEWER STAKES: 1. Provide staking necessary for construction of structures and pipes as shown on plans. Stakes provided one time only. a. Provide graded offset hub and lath area intakes and manholes, offsets to centerline intakes and manholes with grades to invert and rim elevations. Page 2 of 3 Project #418110-0 | January 31, 2019 b. Provide graded offset hub and lath for pipes, offsets to centerline pipe with grades to invert pipe. D. UTILITY STAKES: 1. Provide staking necessary for construction of apparatuses and pipes as shown on plans. Stakes provided one time only. a. Provide graded offset hub and lath for hydrants, offsets to centerline hydrant with grades to finish surface. b. Provide line stakes for waterline with lath only, marking angle points and tangent lines at 50 foot intervals. Provide grades at critical crossings. c. Provide graded offset hubs and lath for transformer pad with grades to finish surface. d. Provide line stakes for electric with lath only, marking angle points and tangent lines at 50 foot intervals. E. GPS MACHINE CONTROL FOR MASS GRADING: a. Provide control points necessary for construction of sub-grade of road and site grading using GPS machine control. Stakes provide one time only 1) Provide secondary control monuments. Approximately five (5) points. 2) Provide grade check stakes (approximately 10 stakes per event) to check machine control for sub-base and top of rock. F. PAVING STAKES: a. Provide staking necessary for finish grading of paving and drive areas. Stak es provided one time only. 1) Provide graded offset hub and lath for paving and driveways, offsets to back of curb or edge of concrete with grades to finish surface at angle points, radius points, grade breaks, 25 foot intervals on tangent lines and lines of curvature, and intermediate points in parking areas. G. SIDEWALK STAKES: a. Provide staking necessary for sidewalks. Stakes provided one time only. 1) Provide graded offset hub and lath on one side only for sidewalks, offsets to edge of walk with grades to finish surface at angle points, radius points, grade breaks, 25 foot intervals on tangent lines and lines of curvature, and intermediate points. H. BUILDING AND STRUCTURE: a. Provide staking necessary for construction of building pad and building footings and structures as defined in plans. Stakes provide one time only. 1) Provide graded offset hub and lath for construction of building pad, offsets to corners of building pad with grades to finish floor building. 2) Provide benchmark to finish floor elevation at locations of n ew building construction. I. PLAY EQUIPMENT STAKES: a. Provide staking necessary for play equipment. Stakes provided one time only. Page 3 of 3 Project #418110-0 | January 31, 2019 1) Provide graded offset hub and lath for support post, offsets to centerline post with grades to finish surface. J. CONSTRUCTION STAKING SHALL BE PROVIDED FOR $7,500. SCHEDULE We will start our services upon receipt of this executed Agreement. COMPENSATION: We will provide the Scope of Services for fixed fee of thirteen thousand dollars ($13,000.00) It should be noted that during the process, unexpected items or events occur that sometimes result in the need for additional services that cannot be identified in the proposa l stage. Should additional services be required beyond the Scope of Services listed in this proposal, we will submit an other amendment to our contract for your approval and signature prior to our proceeding with the additional services. The terms of this proposal are valid for 30 days from the date of this proposal. TERMS AND CONDITIONS No changes from the terms and conditions agreed-upon in the agreement dated October 15, 2018. AGREEMENT When accepted by both parties, this Amendment will amend the Original Agreement and is subject to all other terms and conditions of the Original Agreement. Original, facsimile, electronic signatures or other electronic acceptance by the parties (and returned to Shive-Hattery) are deemed acceptable for binding the parties to the Amendment. The Client representative signing this Amendment warrants that he or she is authorized to enter into this Amendment on behalf of the Client. Thank you for considering this amendment.
