AGENDA ITEM:Consideration of approval of Change Order No. 3 to Caliber Concrete, LLC,
for the Fox Creek Park Phase 2 Project increasing the contract in the amount
of $12,109.00
FORMAT:Consent Agenda
SYNOPSIS INCLUDING PRO & CON: Change Order No. 3 to Caliber Concrete in the amount of
$12,109.00 reflects cost adds to correct items identified during the Building
Official’s code review, as well as items discussed in progress meetings to
date, such as: changing the storage room ceiling type, altering the play tunnel
design for improved constructability and safety, adding brackets and
galvanization to the transformer gates, and modifying bathroom walls/ceilings
to work with framing. Representative from Genus and City Staff have
reviewed the quantities and find them to be accurate.

Dear Mr. Jermier:
Please find enclosed Change Order No. 3 in the amount of $12,109.00.
This Change Order reflects cost adds to correct items identified during
the Building Official’s code review, as well as items discussed in
meetings to date, such as: changing the storage room ceiling t ype,
altering the play tunnel design for improved constructability and
safety, adding brackets and galvanization to the transformer gates,
and modifying bathroom walls/ceilings to work with framing. The
amount listed above is the net dollar value of the added costs that
comprise Change Order No. 3. The quantities and changes completed
to date have been reviewed and agreed upon by the contractor and
we hereby recommend that the City of Waukee approve Change
Order No. 3.
