AGENDA ITEM:Consideration of approval of a resolution approving Request for Additional
Services No. 1 with Genus Landscape Architects for the Fox Creek Park
Phase 2 Project in the amount of $5,350.00.
FORMAT:Consent
SYNOPSIS INCLUDING PRO & CON: The Request for Additional Services is related to easement
acquisition for the electrical service into the park, site plan revisions as a
result of the Iowa Department of Public Health’s (IDPH) review of the splash
pad. These changes, as required by MidAmerican Energy and IDPH were
necessary to keep the project moving forward and on schedule but resulted in
work not planned for in the original contract scope. Additional, reimbursable
expenses were incurred beyond those anticipated when the initial
reimbursement allowance was set. These include IDPHA permit fees and
additional survey to capture the as-built condition of the sidewalk/drive at the
east entry to the site.

RESOLUTION 18-
APPROVING REQUEST FOR ADDITIONAL SERVICES NO. 1 WITH GENUS
LANDSCAPE ARCHITECTS FOR THE FOX CREEK PARK PHASE 2 PROJECT
IN THE NAME AND BY THE AUTHORITY OF THE CITY OF WAUKEE, IOWA
WHEREAS, the City of Waukee, Dallas County, State of Iowa, is a duly organized Municipal
Organization; AND,
WHEREAS, on February 20, 2018, the Waukee City Council approved an Agreement for
Professional Consulting Services with Genus Landscape Architects related to the design
development and construction and bid administration for Fox Creek Park Phase 2 (Resolution
#18-075); AND,
WHEREAS, the City proposes an Additional Services Agreement No. 1 related to easement
acquisition for the electrical service into the park and site plan revisions as a result of the Iowa
Department of Public Health’s (IDPH) review of the splash pad; AND,
WHEREAS, the agreement, attached hereto as Exhibit A, has been reviewed and approved by
the Waukee City Attorney.
NOW THEREFORE BE IT RESOLVED by the City Council of the City of Waukee that the
Request for Additional Services No. 1 with Genus Landscape Architects for the Fox Creek Park
Phase 2 Project is hereby approved.
Passed by the City Council of the City of Waukee, Iowa, and approved the 5th day of November,
2018.

Dear Mr. Jermier,
This letter is to request fees for additional services related to easement
work and site plan changes as a result of Iowa Department of Public
Health’s (IDPH) review of the splash pad. These changes, as required by
MidAmerican Energy and IDPH were necessary to keep the project
moving forward and on schedule but resulted in work not planned for in
the original contract scope. Additionally, reimbursable expenses were
incurred beyond those anticipated when the initial reimbursement
allowance was set. These include IDPH permit fees and additional
survey to capture the as-built condition of the sidewalk/drive at the east
entry to the site. At this time, $1,179.29 remains in the reimbursable
budget to account for design team mileage during Construction
Observation.
