AGENDA ITEM:Consideration of approval of Change Order No. 2 to Concrete Connections,
for the Waukee Trailhead and Dog Park project increasing the contract in the
amount of $1,800.00.
FORMAT:Consent Agenda
SYNOPSIS INCLUDING PRO & CON: Change Order No. 2 to Concrete Connection is for the
installation of a PCC Flume in the front detention basin at the entrance into
the dog park. The area has minimal slope and standing water has been present
in this area the entire summer. The installation of the a PCC flume will allow
the water to flow at minimal slop and will allow for the native seeding and
erosion matting to be installed. This option will additionally provide the city
with a long-term solution to standing water at entrance to the park. The flume
is approximately 90’ long and 3’ wide.
Change order No. 2 also adjusts the Substantial Completion Date from
October 15, 2018 to December 15th, 2018. This is to account for the wet fall
that we have encountered. Attempts at seeding and planting have been
delayed multiple times.

DESCRIPTION:
During construction, we identified some grading connections that would make the project and surrounding properties drain better. Secondly the
concrete walkway on the West end was significantly higher than design grades exhibited. To adjust sidewalk elevations and connections to the
dog park this area needed to be re-worked.
Original Contract: $ 639,327.50
