SYNOPSIS INCLUDING PRO & CON:
The change order is for additional work to implement temporary
stabilization measures across Sugar Creek Golf Course until final surface
restoration is completed by the City
FISCAL IMPACT INCLUDING COSTBENEFIT ANALYSIS: The net result of this
Change Order increases the contract price by $5,145.00.

CHANGE ORDER NO. 4
Attached are three copies of Change Order No. 4 for the Northwest Area Trunk Sewer
Extension — Phase 1 project under the contract between the City of Waukee and Rognes
Corp.
Change Order No. 4 is for additional work to implement temporary stabilization measures
across Sugar Creek Golf Course until final surface restoration is completed by City.
Change Order No. 4 increases the contract price by a total of $5,145.00. Veenstra &
Kimm, Inc. recommends approval of Change Order No. 4.
Please sign all copies of Change Order No. 4 in the spaces provided. Return one signed
copy of Change Order No. 4 to our office and one signed copy to Rognes Corp.
