Lyon - MidAmerican ENERGY OBSESSIVELY, RELENTLESSLYAT YOUR SERVICE. 2/23/2017 City of Waukee Attention: Rudy Koester 805 University Ave, Waukee, IA 50263 MidAmerican Energy 10510 Douglas Ave, Urbandale, IA 50322 Reference: Three-phase overhead electric service to serve the Alice Rd Lift Station at 633 SE Alice's Rd (WMIS #2592164) Dear Mr. Koester: The enclosed drawing shows MidAmerican Energy Company's proposal to provide a three-phase overhead transformer bank for the above project. The applicant charge for this installation is $224.90. This proposal is valid for 90 days and if MidAmerican Energy Company construction has not commenced within 12 months it may be voided. This proposal is subject to the following terms: MidAmerican Energy Company Proposes To: 1. Furnish and install an overhead primary line extension as shown on the drawing complete with poles, anchors, primary conductors, and a 277/480 volt, three-phase, four -wire overhead transformer bank. 2. Furnish and install up to fifty -feet (50) of overhead service conductor. If you choose to install underground service from the meter socket to MidAmerican Energy's pole, there will be a charge for the secondary riser (This cost is the applicant charge shown above). 3. Furnish instrument transformers for metering which are to be picked up at the MidAmerican Energy Company storeroom. 4. Furnish and install the appropriate electric meters (for each tenant). The Applicant Agrees To: 1. Furnish and install an electric meter setting for each tenant, applicable sockets, conduit, cabinets and wiring according to MidAmerican Energy Company's standards. 2. Install metering instrument transformers sized and furnished by MidAmerican Energy Company and furnish and install the meter test switch for all services above 400 Amp. 3. Locate all underground facilities such as storm and sanitary sewer, septic lines, underground electric cable, communication cable, irrigation systems and water lines that are not located by members of One Call. MidAmerican Energy Company assumes no Page 2 liability for private facilities not located. 4. Furnish easements to MidAmerican Energy Company without cost. (Furnish survey drawing and/or legal description for easement preparation.) If a Yd party easement is required any cost associated in obtaining the easement will be paid by the applicant. All meter and related metering equipment locations shall be approved by the company. Unless Company approval of an exception is given, all meters and related meter equipment must be installed outside and securely attached to permanent structure. MidAmerican Energy Company installed facilities will remain the property of MidAmerican Energy Company. If MidAmerican Energy Company is required to start construction of underground electric facilities during the winter season, the work will be subject to an additional winter construction charge. The undersigned certifies that the structure served by the electric distribution system meets the energy conservation standards as contained in the Iowa Administrative Code Section 661-16.800 sometimes known as the "State Building Code Thermal and Lighting Efficiency Standards." If this proposal is satisfactory, please sign and return one (1) copy to me. MidAmerican Energy Company will release the work for scheduling upon receipt of a signed proposal, payment and easement.
