Company Proposal [Alice's Rd Widening Improvements Phase 4]

SYNOPSIS INCLUDING PRO & CON: MidAmerican Energy will be relocating a transformer at
1100 Alice's Rd (Gilcrest Jewett) to allow for construction as part of the
Alice's Rd Widening Improvements Phase 4 project.
FISCAL IMPACT INCLUDING COSTBENEFIT ANALYSIS: $15,070.00

APPROVING MIDAMERICAN ENERGY COMPANY PROPOSAL
[ALICE'S ROAD CORRIDOR PHASE 4 PROJECT, TRANSFORMER RELOCATION]
IN THE NAME AND BY THE,4 UTHORITY OF THE CITY OF WAUKEE, IOWA
WHEREAS, the City of Waukee, Dallas County, State of Iowa, is a duly organized Municipal
Organization; AND,
WHEREAS, the City is currently construction public improvements as part of the Alice's Road
Corridor Phase 4 Project; AND,
WHEREAS, as part of the project, MidAmerican Energy Company has provided a proposal,
attached hereto as Exhibit A, in order to relocate a transformer at 1100 SE Alice's Road; AND,
WHEREAS, City staff recommends approval of the agreement.
NOW THEREFORE BE IT RESOLVED by the City of Waukee City Council in session this
10`h day of July, 2017, that it hereby approves the MidAmerican Energy Company proposal
[Alice's Road Corridor Phase 4 Project, Transformer Relocation].

Reference: Relocation of the 300KVA transformer that serves the above address
WMIS#2619667
Dear Customer:
The enclosed drawing shows MidAmerican Energy Company's proposal to relocate the
transformer at the above address. The applicant charge for this work is $15,070.00. This
proposal is valid for 90 days and if MidAmerican Energy Company construction has not
commenced within 12 months it may be voided.
It is MidAmerican Energy Company's responsibility to see that the various utility
companies' facilities are located before our construction. This includes electric, natural
gas, telephone, cable television, and generally water. It is the owner's responsibility to
see that any privately owned systems such as water systems, irrigation systems, drain
pipes, septic lines and underground wiring are located before MidAmerican Energy
Company's construction. MidAmerican Energy Company assumes no liability for
private facilities that are not located.
If this proposal is satisfactory please sign and return one (1) copy to me. Please include
payment of $15,070.00, MidAmerican Energy Company will release the work upon
receipt of the signed proposal and payment. Please call me at (515) 252-6472 if you have
