AGENDA ITEM: Consideration of approval of Change Order No. 3 to Visu-Sewer, Inc. for
the Original Sanitary Sewer Collection System Rehabilitation — Phase
2A Project increasing the contract in the amount by $76,450.00
FORMAT: Motion to Approve.
,SYNOPSIS INCLUDING PRO & CON: The change order is to for the epoxy lining of 13
manholes located at the discharge end of 5 force mains throughout the
City. Change Order No. 3 also modifies the project completion date
from April 15, 2017 to November 3, 2017 and modifies the damages
from $500.00 to $1,000.00 per calendar day for each day that work shall
remain uncompleted after November 3, 2017.
FISCAL IMPACT INCLUDING COSTBENEFIT ANALYSIS: The net result of this
Change Order increases the contract.price by $76,450,00,

CHANGE ORDER NO. 3
Enclosed are three copies of Change Order No. 3 for the Original Sanitary Sewer Collection
System Rehabilitation - Phase 2A project under the contract between the City of Waukee and
Visu-Sewer, Inc.
Change Order No. 3 is for the epoxy lining of 13 manholes located at the discharge end of 5
force mains throughout the City.
Change Order No. 3 increases the contract price by a total of $76,450.00. Change Order No.
3 also modifies the project completion date from April 15, 2017 to November 3, 2017 and
modifies the damages from Five Hundred Dollars ($500.00) to One Thousand Dollars
($1,000.00) per calendar day for each day that work shall remain uncompleted after
November 3, 2017. Veenstra & Kimm, Inc. recommends approval of Change Order No. 3.
Please execute all three copies of Change Order No. 3 in the spaces provided. Return one
copy to Veenstra & Kimm, Inc., forward one copy to Visu-Sewer, Inc., and retain one copy for
your files.
