AGENDA ITEM: Consideration of approval of a resolution approving Supplemental Agreement
with Snyder & Associates, Inc. [Windfield/High School Trail — Phase 1]
FORMAT: Resolution
SYNOPSIS INCLUDING PRO & CON: The original agreement with Snyder & Associates was for
the design portion of the project. The Supplemental Agreement is for
construction administration services that includes a preconstruction
conference, contractor payment requests, shop drawings review, processing
change orders, and a substantial completion final site observation.
FISCAL IMPACT INCLUDING COSTBENEFIT ANALYSIS:
Not to exceed budget of $4,900
COMMISSIONBOARD/COMMITTEE COMMENT:
STAFF REVIEW AND COMMENT: Approve agreement.
RECOMMENDATION: Approve the resolution.

APPROVING SUPPLEMENTAL AGREEMENT FOR ADDITIONAL SERVICES WITH
SNYDER & ASSOCIATES, INC. [WINDFIELD/HIGH SCHOOL TRAIL PHASE 1
PROJECT]
IN THE NAME AND BY THE AUTHORITY OF THE CITY OF WA UKEE, IOWA
WHEREAS, the City of Waukee, Dallas County, State of Iowa, is a duly organized Municipal
Organization; AND,
WHEREAS, on May 8, 2017, the Waukee City Council approved an Agreement for
Professional Consulting Services with Snyder & Associates, Inc. related to engineering services
for the Windfield-High School Trail Project (Resolution 417-232); AND,
WHEREAS, Snyder & Associates, Inc., has since provided the City with a Supplemental
Agreement for Additional Services, attached hereto as Exhibit A, related to construction
administration servised;
NOW THEREFORE BE IT RESOLVED by the City Council of the City of Waukee that the
Supplemental Agreement for Additional Services with Snyder & Associates, Inc. [Windfield/
High School Trail Phase 1 Project] is hereby approved.
Passed by the City Council of the City of Waukee, Iowa, and approved the 18`h day of
September, 2017.

This is authorization for Snyder & Associates, Inc. to proceed with the following described additional services.
Client: City of Waukee
Project Name: Windfield/High School Trail - Phase I
S&A Project Number: 117.0519.01 Date: September 13, 2017
DESCRIPTION OF ADDITIONAL SERVICES:
Upon award of the construciton contract, Snyder & Associates, Inc. will perform construction
administration services for the project including:
1. Preconstruction Conference: Snyder & Associates, Inc. will arrange and conduct a preconstruction
conference with the Contractor, Subcontractors and the City to review the contract requirements, details
of construction, utility conflict and work schedule prior to construction.
2. Contractor Payment Requests: Requests for progress payments will be reviewed and approved based
on site observations, which authorizes payments and is a declaration that the contractor's work has
progressed to the point indicated.
3. Shop Drawings: Shop drawings, if any and other submissions of the Contractor will be reviewed for
general compliance with the construction contract.
4. Change Orders: Change Orders will be prepared for approval by the City.
5. Substantial Completion Final Site Observation: Snyder & Associates, Inc. shall perform a site
observation to determine if the project is substantially complete according to the plans and specifications
and make recommendation on final payment.
Lump Sum in the amount of: $

The undersigned, on behalf of the Client, understands and agrees that the services described in this
Supplemental are additional service, scope of which is not contained within the original scope of services
defined in the original agreement. The Additional Services in this Supplemental are subject to the general
conditions contained in the original Professional Services Agreement dated
