STAFF REVIEW AND COMMENT: Veenstra & Kimm, Inc. has reviewed public improvements
for Fox Creek Trunk Sewer — Phase 3 and find it in general conformance with
the standard specification for the City of Waukee.
RECOMMENDATION: Approve the resolution.

RESOLUTION 17-
RESOLUTION ACCEPTING THE FOX CREEK TRUNK SEWER - PHASE 3
WHEREAS, on the 6 h day of March, 2017, the Mayor and Clerk of the City of Waukee,
Iowa, entered into a construction contract with Graff Excavating of Toledo, IA, for the
construction of certain public improvements generally described as the Fox Creek Trunk Sewer -
Phase 3; and
WHEREAS, the contractor has fully completed the construction of the public
improvements in accordance with the terms and conditions of the contract and plans and
specifications, as shown by the certificate of the Engineer filed with the Clerk on November 1,
2017:
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY
OF WAUKEE, IOWA:
Section 1. That the report of the Engineer be and the same is hereby approved and
adopted, and the public improvements are hereby approved and accepted as having been fully
completed in accordance with the plans, specifications and form of contract and the total final
construction cost thereof is hereby determined to be $583,888.00, as shown in the report of the
Engineer.

CERTIFICATE OF COMPLETION
Enclosed are two copies of the Certificate of Completion for the Fox Creek Trunk
Sewer — Phase 3 project. The certificate should be executed after the City Council
has approved acceptance of the project. Please sign both certificates and return one
signed copy to our office.

We hereby certify that we have made an on -site review of the completed
construction of the Fox Creek Trunk Sewer — Phase 3 project as performed by Graff
Excavating, Inc.
As Engineers for the project, it is our opinion the work performed is in
substantial accordance with the plans and specifications, and that the final
amount of the Contract is Five Hundred Eighty-three Thousand Eight Hundred
Eighty-eight and 00/100 Dollars ($583,888.00).
