STAFF REVIEW AND COMMENT: Veenstra & Kimm, Inc. has reviewed public improvements
for the Original Sanitary Sewer Collection System Rehabilitation — Phase 2A
Project and find it in general conformance with the standard specification for
the City of Waukee.
RECOMMENDATION: Approve the resolution.

RESOLUTION ACCEPTING THE ORIGINAL SANITARY SEWER
COLLECTION SYSTEM REHABILITATION - PHASE 2A
WHEREAS, on the 18t' day of July, 2016, the Mayor and Clerk of the City of Waukee,
Iowa, entered into a construction contract with Visu-Sewer of Pewaukee, WI, for the
construction of certain public improvements generally described as the Original Sanitary Sewer
Collection System Rehabilitation - Phase 2A; and
WHEREAS, the contractor has fully completed the construction of the public
improvements in accordance with the terms and conditions of the contract and plans and
specifications, as shown by the certificate of the Engineer filed with the Clerk on November 1,
2017:
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY
OF WAUKEE, IOWA:
Section 1. That the report of the Engineer be and the same is hereby approved and
adopted, and the public improvements are hereby approved and accepted as having been fully
completed in accordance with the plans, specifications and form of contract and the total final
construction cost thereof is hereby determined to be $395,856.95, as shown in the report of the
Engineer.

CERTIFICATE OF COMPLETION
Enclosed are two copies of the Certificate of Completion for the Original Sanitary Sewer
Collection System Rehabilitation - Phase 2A project. The certificate should be executed
after the City Council has approved acceptance of the project. Please sign both certificates
and return one signed copy to our office.

We hereby certify that we have made an on -site review of the completed
construction of the Original Sanitary Sewer Collection System Rehabilitation - Phase
2A as performed by Visu-Sewer, Inc.
As Engineers for the project, it is our opinion the work performed is in
substantial accordance with the plans and specifications, and that the final
amount of the Contract is Three Hundred Ninety -Five Thousand Eight Hundred
Fifty -Six and 95/100 Dollars ($395,856.95).
