SYNOPSIS INCLUDING PRO & CON:
The change order is for:
a. Additional payment to remove and reinstall handholes and conduit at Ashworth Road due to
water main construction
b. Increase quantity of wire listed in the Schedule of values for signal to operate per plan
c. Additional square footage to pay for signage switch from "SE Alices Rd" to "Grand Prairie
Pkwy"
d. Addition of two illuminated "No Right on Red" signs at the intersection of SE University
Ave and SE Alices Road
e. Addition of two "Stop Here on Red" signs on Ashworth Road
FISCAL IMPACT INCLUDING COST/BENEFIT ANALYSIS:
increase the contract by $40,230.35.

Dear Mr. Gibson:
Attached please find a detailed Change Order for City Council approval on January 18, 2016.
Change Description
Additional payment to remove and reinstall handholes and conduit at Ashworth Road due to water main
construction.
Increase quantity of wire listed in the Schedule of values for signal to operate per plan.
Additional square footage to pay for signage switch from "SE Alices Rd" to "Grand Prairie Pkwy".
Addition of two Illuminated "No Right on Red" signs at the intersection of SE University Ave & SE
Alices Rd.
Addition of two "Stop Here on Red" signs on Ashworth Road.
We have reviewed the submitted quotes from Howrey Construction and Iowa Signal and they appear to
be in order with current industry pricing. If City Staff concurs with this pricing, it is recommended the
City of Waukee increase the contract price in the amount of 40$ ,230.35 to complete the work associated
with Change Order #04.

You are hereby authorized to make the following changes to the contract documents.
A - Description of change to be made or extra work to be done:
7004 - Increase quantity of Traffic Signalization by $40,230.35
B - Reason for change or extra work:
7004 - Increase in amount is due to five components:
1) During construction, handholes and conduit needed to be removed then reinstalled for circumstances contractor had no control of.
2) Quantity of Detector Cable and Deteector Lead-in Cable listed on the schedule of unit values was significantly lower than of the wire needed
for the signal to appropriately detect vehicle movements.
3) Additional Sq Ft for switching road sign from "SE Alices Rd" to "Grand Prairie Parkway'.
4) Installation of two Illuminated "No Right on Red" signs at the intersection of SE University Ave and BE Alices Rd.
5) Installation of two "Stop here on Red" signs on BE Ashworth Rd.
