STAFF REVIEW AND COMMENT: Veenstra & Kimm, Inc. has reviewed the Original Sanitary
Sewer Collection System Rehabilitation Phase 1 and find it in general
conformance with the standard specification for the City of Waukee.
RECOMMENDATION: Approve the resolution.

RESOLUTION ACCEPTING THE ORIGINAL SANITARY SEWER
COLLECTION SYSTEM REHABILITATION - PHASE 1
WHEREAS, on the 15`h day of June, 2015, the Mayor and Clerk of the City of Waukee,
Iowa, entered into a construction contract with Hydro-Klean, LLC, of Des Moines, IA, for the
construction of certain public improvements generally described as the Original Sanitary Sewer
Collection System Rehabilitation - Phase 1; and
WHEREAS, the contractor has fully completed the construction of the public
improvements in accordance with the terms and conditions of the contract and plans and
specifications, as shown by the certificate of the Engineer filed with the Clerk on May 31, 2016,
2016:
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY
OF WAUKEE, IOWA:
Section 1. That the report of the Engineer be and the same is hereby approved and
adopted, and the public improvements are hereby approved and accepted as having been fully
completed in accordance with the plans, specifications and form of contract and the total final
construction cost thereof is hereby determined to be $173,412.68, as shown in the report of the
Engineer.

Enclosed are two copies of the Certificate of Completion for the Original Sanitary Sewer
Collection System Rehabilitation - Phase 1 project. The certificate should be executed
after the City Council has approved acceptance of the project. Please sign both certificates
and return one signed copy to our office.

We hereby certify that we have made an on -site review of the completed
construction of the Original Sanitary Sewer Collection System Rehabilitation -
Phase 1 as performed by Hydro-Klean LLC.
As Engineers for the project, it is our opinion the work performed is in
substantial accordance with the plans and specifications, and that the final
amount of the Contract is One Hundred Seventy -Three Thousand Four Hundred
Twelve and 68/100 Dollars ($173,412.68).
