AGENDA ITEM: Consideration of approval of Change Order No. 1 to Hawkeye Paving
Corporation for the NE Douglas Parkway Extension, From NE Alices Rd
west 560-ft increasing the contract in the amount of $76,743.03
FORMAT: Consent Agenda
SYNOPSIS INCLUDING PRO & CON:
The change order is for the following items:
• Additional 30 LF of 54-inch storm sewer to extend the pipe farther south and west to
provide a lower flowline for the adjacent development
• Adding 28 LF of CMP culvert to extend existing 24-inch pipe under Alices Road
Additional 637.97 SY of pavement widening to account for patching Alices Road
• Additional 637.97 SY of pavement removals to account for patching Alices Road
+ Adding 17 EA of CD baskets for patching along Alices Road that required intermediate
joints
+ Adding 20 STA of Waterborne Painted Pavement Markings to temporary reestablish the
centerline striping during construction
FISCAL IMPACT INCLUDING COST/BENEFIT ANALYSIS: The net result of this
Change Order increases the contract price by $76,743.03. The City of
Clive reimbursement to the City of Waukee for Change Order #01 is
$32,618.68

Dear Mr. Gibson:
Attached please find a detailed Change Order for City Council approval on June 20, 2016.
Chance Description
• Additional 30 LF of 54-inch storm sewer to extend the pipe farther south and west to provide a
lower flowline for the adjacent development.
• Adding 28 LF of CMP culvert to extend existing 24-inch pipe under Alices Road.
• Additional 637.97 SY of pavement widening to account for patching Alices Road.
• Additional 637.97 SY of pavement removals to account for patching Alices Road.
• Adding 17 EA of CD baskets for patching along Alices Road that required intermediate joints.
• Adding 20 STA of Waterborne Painted Pavement Markings to temporary reestablish the
centerline striping during construction.
We have reviewed the submitted quotes from Hawkeye Paving Corporation and it appears to be in order
with current industry pricing. If City Staff concurs with this pricing, we recommend the City of Waukee
increase the contract price in the amount of $76,743.03 to complete the additional work associated with
Change Order #01. The City of Clive reimbursement to the City of Waukee for Change Order #01 is
$32,618.68
