for the Centennial Park Creek Slope Stabilization project increasing the
contract in the amount of $23,595.00

SYNOPSIS INCLUDING PRO & CON:
The change order is for the following items:
• Additional mobilization to perform additional bank stabilization
• Additional tree removal
• Additional channel excavation to repair bank from SE University Ave to downstream
riffle
• Additional seeding to do the additional bank repair
• Additional tree installation to account for the additional removals
• Removal of various bid items as they were not used during construction
• Additional Class E revetment for bank repair from SE University Ave to downstream
riffle
• Extension of contract to perform additional repairs from SE University Ave to
downstream riffle
FISCAL IMPACT INCLUDING COSTBENEFIT ANALYSIS: The net result of this
Change Order increases the contract price by $23,595.00.

Dear Mr. Gibson:
Attached please find a detailed Change Order for City Council approval on June 20, 2016.
Change Description
• Additional mobilization to perform additional bank stabilization.
• Additional tree removal.
• Additional channel excavation to repair bank from SE University Ave to downstream riffle
• Additional seeding to due to additional bank repair
• Additional tree installation to account for the additional removals
• Removal of various bid items as they were not used during construction
• Additional Class E revetment for bank repair from SE University Ave to downstream riffle
• Extension of contract to perform additional repairs from SE University Ave to downstream riffle
We have reviewed the request for additional mobilization from TK Concrete, Inc. and it appears to be in
order with current industry pricing; all other items will use established contract prices. If City Staff
concurs with this pricing, we recommend the City of Waukee increase the contract price in the amount of
$23,595.00 to complete the additional work associated with Change Order #01.
