SYNOPSIS INCLUDING PRO & CON:
The change order is for the following items:
Additional costs to raise the manhole approximately I-ft due to lowering of the proposed
54" storm sewer
FISCAL IMPACT INCLUDING COSTBENEFIT ANALYSIS: The net result of this
Change Order increases the contract price by $3,600.00. The City of
Clive reimbursement to the City of Waukee for Change Order #02 is
$0.00

Dear Mr. Gibson
Attached please find a detailed Change Order for City Council approval on July 18, 2016.
Change Description
• Additional costs to raise the manhole approximately 1-ft due to lowering of the proposed 54"
storm sewer.
We have reviewed the submitted quotes from Hawkeye Paving Corporation and it appears to be in order
with current industry pricing. If City Staff concurs with this pricing, we recommend the City of Waukee
increase the contract price in the amount of 3$ ,600.00 to complete the additional work associated with
Change Order #02. The City of Clive reimbursement to the City of Waukee for Change Order #02 is
$0.00.
