Hickman Road] FORMAT: Resolution SYNOPSIS INCLUDING PRO & CON: Waukee Central, LLC is requesting approval of a site plan for an approximately 3,600 square foot dental clinic to be located at the southeast corner of Dartmoor Drive and Hickman Road directly east of the proposed Central Bank. Access to the site will be provided entirely off of a private street [Laurel Street] which is being developed as a part of the Waukee Central Development with shared access with the bank. FISCAL IMPACT INCLUDING COST/BENEFIT ANALYSIS: COMMISSION/BOARD/COMMITTEE COMMENT: The Planning and Zoning Commission reviewed the request at their meeting on July 12, 2016 and recommended approval. Approval of a Site Plan for Waukee Dental (350 E Hickman Road) City Planner Melissa DeBoer introduced the request made by applicants Waukee Central LLC for approval of the Site Plan for Waukee Dental. Mrs. DeBoer advised that the subject property is located at the southeast corner of Hickman Road and SE Dartmoor Drive and is approximately 0.88 acres in size and would hose the proposed 3616 square foot building. Access to the site will be provided off of the private street located to the South of the site and a shared ingress/egress easement on the west side of the site. A total of 11 parking spaces are required for this facility. The total amount of parking proposed is 26 spaces including 2 handicap accessible stalls. Traffic throughout the parking lot will be two-way traffic. A five foot wide sidewalk will be installed along SE Laurel Street located on the south side of the site as well as internally on the site primarily around the building from the parking area. All utilities will be extended to this site during platting with sanitary sewer and water being extended from the south of the property. Storm water detention will be provided with a dry bottom detention basin located on the north side of the site. The proposed amount of open space is 32% of the project area which satisfies the requirements of 20% being dedicated to open space. The applicant has provided the required amount of planting per the landscaping ordinance and has proposed a small berm to add additional screening to the trash enclosure.  Commissioner Nigro questioned who would have the weight of liability on the private street. Mr. Deets stepped in and advised that the City would not be held liable as it was a private street, the weight of responsibility would fall on the commercial association.  Commissioner Fontenot questioned if the private street would be completed all at once and Mr. Deets advised that yes, the street would be completed in the early stages of building to provide access off of Dartmoor Drive and NE Westgate Drive. Motion to approve a Site Plan for Waukee Dental (350 E. Hickman Road) subject to any remaining staff comments and completion of legal documents was moved by Commissioner Fontenot Seconded by Commissioner Streit. Ayes: Nigro, Fontenot, Streit, Broderick, Hoifeldt, Shanks, and Schmidt. Nays: none. Motion carried. The property in question is approximately 0.88 acres. The applicant, Waukee Central, LLC, is requesting approval of a site plan for a dentist office. July 8, 2016 2 of 3 ABOVE: Aerial of the site plan (outlined in RED) in relation to the surrounding properties. Shaded in GRAY is the proposed bank to the east. PROJECT DESCRIPTION: The proposed project includes the construction of a 3,616 square foot building to be used for a dentist office. ACCESS AND PARKING: Access to the site will be provided off of the private street located to the south of the site. A shared ingress/egress easement on the west side of the site will also provide access between this site and the site located to the west. A total of 11 parking spaces are required for this facility. The total amount of parking proposed is 26 spaces (including 2 handicap accessible stalls). Traffic throughout the parking lot will be two-way traffic. SIDEWALKS/TRAILS: A five foot wide sidewalk will be installed along SE Laurel Sreet, located on the south side of this site. Sidewalks are planned internal to the site, primarily around the building from the parking areas. The site plan does provide a pedestrian connection from the site to the sidewalk that will be constructed along the north side of SE Laurel Street. UTILITIES: All utilities will be extended to this site during platting. Sanitary sewer will be extended from SE Laurel Street located to the south. Water will also be extended from the south. July 8, 2016 3 of 3 Storm water detention will be provided with a dry bottom detention basin located on the north side of the site. OPEN SPACE AND LANDSCAPING: A minimum of 20% of the project area is required to be open space. The total project area is 0.88 acres and the proposed amount of open space is 32% of the project area. The applicant has provided the required amount of plantings per the landscaping ordinance. ELEVATIONS: The proposed building will be constructed mostly of brick and stone. MISCELLANEOUS: A trash enclosure is proposed on the southeast corner of the site. A small berm has been provided along the south side of the trash enclosure to provide additional screening of the trash enclosure from the private street. The applicant has indicated areas of future parking on both the east and west sides of the building. For now, these areas will be sodded. The applicant has provided a lighting plan which meets the City of Waukee Site Plan Ordinance requirements. STAFF RECOMMENDATION: At this time staff is comfortable with the site plan and would recommend approval of the site plan subject to remaining staff comments. The property is located at the southeast corner of SE Dartmoor Dr and Hickman Rd. Based upon our review, staff has the following comments related to the site plan: COVER SHEET & GENERAL COMMENTS 1. Please revise name of plans to read Waukee Dental to be consistent. 2. Provide project manager contact information on the cover sheet. 3. Label more streets within the vicinity map. 4. Revise “Grading Permit” to “COSESCO Permit” in the second note box in the lower right hand corner on the cover sheet. 5. Include the City of Waukee Standard Specifications prior to SUDAS in the last note box in the lower right hand corner on the cover sheet. Note should reference 2016 edition of SUDAS. 6. Provide the property address on the cover sheet (350 E Hickman Rd). 7. Please provide a materials board with the next submittal. This must be submitted prior to forwarding to Planning and Zoning Commission. 8. Provide details of any exterior signage proposed. Verify all book and page numbers shown are accurate. 14. Staff questions if any septic systems exist on the property. If so, provide a note to abandon and remove per IDNR and Dallas County Environmental Health Department requirements. 15. On the notes stating to abandon the well per Iowa DNR requirement, add Dallas County requirements as well. 16. Staff questions if demolition will be done as part of plat improvements or with each individual site. Please address. SHEET 3 | DIMENSION PLAN 17. In addition to the private street label, label the private street shown as SE Laurel St on all sheets. 18. Provide a downspout connection detail and label these locations on the site plan. Show storm sewer service lines on plan. 19. Provide a sign easement for the shared monument sign shown on the plan. 20. Add a note stating that any changes proposed to the site plan during construction shall be approved in writing by the City of Waukee Development Services Department. 21. Add a note stating that the contractor is responsible for any changes made during construction that have not been granted approval in writing by the City of Waukee Development Services Department. 22. Show the building setbacks on the site plan. 23. Provide a sidewalk easement for the sidewalk shown on the south side of the site. 24. Show the warning panel for the pedestrian ramp located on the south side of the site. 25. Staff questions how only half of the driveway will be installed with this site. Staff has concerns that if this project and the bank project don’t end up getting built at the same time, that the entire portion of the driveway won’t be completed for some time and left unfinished. Provide a note stating that the entire driveway needs to be installed with whichever project (this project or the bank) occurs first. SHEET 4 | GRADING PLAN 26. Demonstrate grades meet 2% slope in all unpaved areas. Please provide more of these labels for the unpaved areas throughout the site. Also, please explain/show how the areas shown as future parking will be graded before they are paved. Add note instructing the contractor to grade future parking to the top of curb elevation. Engineers Response: 27. Illustrate 100 year flood elevations and overflow routes for the detention areas. 28. Provide pedestrian facility information for ramps (running ramp grades, spot elevations, cross slope, etc.). SHEET 5 | UTILITY PLAN 29. Show the connection for the 15” RCP storm sewer in the northeast corner. Staff questions if a DOT permit is needed for this connection. 30. Staff questions if gas will be provided to this site. Please address. Engineers Response: 31. Separate the water (domestic and fire) on the outside of the building and provide a valve for each after the separation. 32. Label all critical crossings with top and bottom of pipe elevations. 33. Show hydrant locations and hydrant radius coverage. Per Waukee Fire Department, with fire- flow requirements and available gpm, two hydrants will be required within 400 feet from all portions of the buildings. Currently a hydrant along SE Laurel Street does meet part of the coverage. An additional hydrant needs to be added on the north side of the building to achieve proper coverage. Per Waukee Fire Department, the proposed hydrant on the site of Central Bank does not provide access when considering normal fire scene operations for Central Bank or for Waukee Dental. Relocated hydrant to north side of site along property line between Waukee Dental and Central Bank. Engineers Response: 34. Staff questions if a grease interceptor is proposed. 35. Show where the water on the south side of the site connects to the water main. 36. Correct the spelling error within the Orifice Plate Detail box (to match “grove”). SHEET 6 | LANDSCAPE PLAN 37. Provide a note stating that substitutions are to be approved in writing by the City of Waukee Development Services Department. This note does not appear to have been added to the plan set. Please add. 38. Provide landscaping to screen all ground mounted mechanical units (transformer/condenser units). Please provide a detail on how these units will be screened and show on the landscape plan. 39. Parking lot setback trees should be provided on SE Laurel St as well. Please show these on the plan. 40. Provide more landscaping around the trash enclosure to provide screening. Please revise landscape plan to provide plantings that provide more height in order to provide a more substantial screening of the trash enclosure. 41. Staff suggests adding a berm on the south side of the trash enclosure in addition to more landscaping to provide adequate screening. 42. Provide a note stating that all disturbed areas are to be sodded. ELEVATIONS 43. Please provide the exterior elevation drawings in color (rather than just the rendering). These must be submitted prior to forwarding to Planning and Zoning Commission. 44. Staff questions if there will be any roof-mounted mechanical units. If so, please address how these will be screened. VEENSTRA & KIMM COMMENTS: 45. Sheet 1: Show more existing streets in vicinity map. 46. Sheet 1: Show address of property. 47. As of now the fee is a quarterly fee of $65, but beginning July 1st it will be a $200 one-time fee. Staff encourages paying the one-time fee of $200, rather than the quarterly fee of $65 which will lower the total COSESCO fees paid. Engineers Response: The applicant will be required to provide the following as a part of the completion of the public improvements prior to approval of a final plat: 63. Post Construction What practices considered for this site? 64. Provide a site plan specific to the storm water management facilities of the property indicating the permanent measures utilized by the designer and requirements for maintenance of said facilities. 65. A maintenance covenant will need to be created and provided for review (if different than the existing). 66. As-built plans of all storm water management facilities on site and a certification statement signed by SWPPP designer indicating that facilities were constructed as designed will be required. Staff would request that the revised plans and comment response letter be provided in digital form (PDF) and hard copies with 5 full-sized hard copies delievered to Waukee City Hall. The PDF copy may be emailed. One additional hard copy set including any storm water management materials should be delivered directly to Veenstra and Kimm to Jordan Kappos’ attention.
