AGENDA ITEM: Consideration of approval of a resolution approving YMCA of Greater Des
Moines Corporate Membership Partnership Agreement
FORMAT: Consent Agenda
SYNOPSIS INCLUDING PRO & CON: As a part of the City of Waukee Wellness program, the
Waukee YMCA is offering a corporate rate to each Waukee staff member
who joins or continues membership in the YMCA. The Waukee YMCA will
offer marketing and creative services to the Waukee Safety and Wellness
Team. The collaboration will be on a one-year trial basis and will be
promoted along with other wellness activities to all staff members.

RESOLUTION 16-
APPROVING YMCA OF GREATER DES MOINES CORPORATE MEMBERSHIP
PARTNERSHIP AGREEMENT
IN THE NAME AND BY THE AUTHORITY OF THE CITY OF WAUKEE, IOWA
WHEREAS, the City of Waukee, Dallas County, State of Iowa, is a duly organized Municipal
Organization; AND,
WHEREAS, the Waukee Family YMCA is offering a corporate rate to each Waukee staff
member who joins or continues membership in the YMCA; AND,
WHEREAS, the partnership includes marketing and creative services to the Waukee Safety and
Wellness Team; AND,
WHEREAS, the Waukee Safety and Wellness Team recommends a one-year trial collaboration
with the Waukee Family YMCA, after which the program will be evaluated for its effectiveness
and potential continuation.
NOW, THEREFORE, BE IT RESOLVED by the City Council of the City of Waukee, Iowa
on this 15th day of August, 2016, that the YMCA of Greater Des Moines Corporate Membership
Partnership Agreement, attached hereto as Exhibit A, is hereby approved.

Corporate Membership Partnership Agreement
Membership Details:
The YMCA of Greater Des Moines will waive the $50 joiner fee for new memberships.
Employees must provide employment verification.
Current employees who are members of the YMCA need to contact the YMCA in person to take
advantage of this Corporate Membership. Refunds will not be given for memberships paid prior
to this agreement and/or prior to the employee bringing in their verification.
Corporate Membership is only available through Corporation Payroll Deduction.
Corporation agrees to a minimum $10/month membership reimbursement to employees.
Membership rates are valid at all branches of the YMCA of Greater Des Moines.
If an employee decides to stop his/her membership, he/she will need to follow the YMCA’s
procedures to end the membership.
If an employee of the Corporation leaves the company, his/her membership will be cancelled. It
is the responsibility of the Corporation to notify the YMCA when an employee leaves
employment in order to not be invoiced for that employee’s membership.
Rates may increase with a minimum of 30 days notice.
How to Join:
An employee must present his/her verification of employment to the YMCA front desk
personnel and complete a membership application at any YMCA of Greater Des Moines location.
The employee is responsible for paying for their portion of the first month of membership at the
time of joining the Y.
Invoicing:
Invoices will be sent via email the 1st business day of each month for the current month of
membership.
The first invoice after joining, will include the full membership fee and the initial Employer Rate
for the first month.
Invoices are due by the 25th of that same month.
Corporation is responsible for verifying the invoice and notifying the Y of any changes.
Y will notify Corporation anytime an employee joins, changes their membership or cancels.
Corporation will notify Y with any changes they are made aware of.
