SYNOPSIS INCLUDING PRO & CON: The change order is various quantity adjustments to
account for actual quantities installed in the field.
FISCAL IMPACT INCLUDING COSTBENEFIT ANALYSIS: The net result of this
Change Order decreases the contract price by $11,212.26. The City of
Clive reimbursement to the City of Waukee for Change Order #3 is
$792.82

Attached please find a detailed Change Order for City Council approval on October 17, 2016
Change Description
• Various quantity adjustments to account for actual quantities installed in the field
We have reviewed the submitted requests from Hawkeye Paving Corporation and quantities appear to be
in order with what was installed in the field. If City Staff concurs with these adjustments, we recommend
the City of Waukee decrease the contract price in the amount of $11,212.26 to account for the additional
work associated with Change Order #03. The City of Clive portion of the decrease to the contract
decrease for Change Order #03 is $792.82.
