STAFF REVIEW AND COMMENT: Foth Infrastructure and Environment, LLC has reviewed the
NE Douglas Parkway Extension from NE Alice's Road West 560 Feet and
find it in general conformance with the standard specification for the City of
Waukee.
RECOMMENDATION: Approve the resolution.

RESOLUTION ACCEPTING THE NE DOUGLAS PARKWAY EXTENSION
FROM NE ALICES ROAD WEST 560 FEET
WHEREAS, on the 0 day of April, 2016, the Mayor and Clerk of the City of Waukee,
Iowa, entered into a construction contract with Hawkeye Paving Corp. of Bettendorf, IA, for the
construction of certain public improvements generally described as the NE Douglas Parkway
Extension From NE Alices Road West 560 Feet; and
WHEREAS, the contractor has fully completed the construction of the public
improvements in accordance with the terms and conditions of the contract and plans and
specifications, as shown by the certificate of the Engineer filed with the Clerk on October 17,
2016 2016:
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY
OF WAUKEE, IOWA:
Section 1. That the report of the Engineer be and the same is hereby approved and
adopted, and the public improvements are hereby approved and accepted as having been fully
completed in accordance with the plans, specifications and form of contract and the total final
construction cost thereof is hereby determined to be $958,410.77, as shown in the report of the
Engineer.
PASSED AND APPROVED this 17th day of October, 2016.

We hereby certify that we have made an on -site review of the completed construction of the above
reference project as performed by Hawkeye Paving Corp.
As Construction Administrators for the project, it is our opinion that the work is in substantial
accordance with the plans and specification and that the final amount of the contract is Nine Hundred
Fifty -Eight Thousand Four Hundred Ten and 77/100 Dollars ($958,410.77).
