FISCAL IMPACT INCLUDING COST/BENEFIT ANALYSIS: Compensation for services on an hourly basis for labor plus reimbursable expenses. [3RD STREET WATER TOWER REFURBISHMENT PROJECT] IN THE NAME AND BY THE AUTHORITY OF THE CITY OF WAUKEE, IOWA WHEREAS, the City of Waukee, Dallas County, State of Iowa, is a duly organized Municipal Organization; AND, WHEREAS, on October 5, 2015, the Waukee City Council approved an Agreement for Engineering Services with Stanley Consultants, Inc., related to the 3rd Street Water Tower Refurbishment Project (Resolution #15-345); AND, WHEREAS, on May 2, 2016, the Waukee City Council approved Supplemental Agreement No. 1 (Resolution #16-159) and on November 4, 2016, approved Supplemental Agreement No. 2 (Resolution #16-XX); AND, WHEREAS, the City proposes Supplemental Agreement No. 3 to the Engineering Services Agreement to address construction observation services to assist the City in oversight of communications equipment installation; AND, WHEREAS, the engineering services supplemental amendment, attached hereto as Exhibit A, has been reviewed by the City Attorney, who finds it satisfactory. NOW THEREFORE BE IT RESOLVED by the City Council of the City of Waukee that the Supplemental Agreement No. 3 with Stanley Consultants, Inc. [3rd Street Water Tower Refurbishment Project] is hereby approved. Lyon Rick Peterson 26485 SC3881 1014 SUPPLEMENTAL AGREEMENT NO. 3 This Supplemental Agreement, made and entered into by and between STANLEY CONSULTANTS INC. (CONSULTANT) and CITY OF WAUKEE (CLIENT) amends their agreement of October 5, 2015, for Consultant to provide services to the Client for the 3rd Street Water Tower Refurbishment Project as follows: Background Prior to the 3rd Street Water Tower Refurbishment Project all antennas and cables were removed from the water tower and a temporary communications tower was erected onsite. The 3rd Street Water Tower Refurbishment Project is now complete and the antennas are now ready to be reinstalled on the water tower. New cable standoff T brackets were welded to five of the water tower legs and the shell of tank for the purpose reinstallation of antenna cables, and six new masts were welded to the top of the tank for the purpose of antenna mounting as part of 3rd Street Water Tower Refurbishment Project. Two (2) telecommunications companies, Verizon and US Cellular, have contracts with the CLIENT to mount antennas to the 3rd Street Water Tower. The Client and Dallas County also have communications and telemetry antennas that are being mounted to the 3rd Street Water Tower. All of the antennas, mounts, cables, and power that is being installed on the 3rd Street Water Tower is referred to as COMMUNICATIONS EQUIPMENT. Installation contractors of the COMMUNICATIONS EQUIPMENT are referred to as CONTRACTORS. Scope of Services CONSULTANT’s services will consist of office and field based construction observation services to assist CLIENT in oversight of COMMUNICATIONS EQUIPMENT installation on the 3rd Street Water Tower as follows: 1. Attend preconstruction conference. 2. Review: Review shop drawings, design drawings and other data which CONTRACTORS submit, but only for compatibility with design concept of the 3rd Street Water Tower Refurbishment Project for COMMUNICATIONS EQUIPMENT installation. Such reviews will not extend to CONTRACTORS’ means, methods, techniques, sequences, or procedures of construction or to safety precautions and programs incident thereto. CONSULTANT will not review technical submittals for the COMMUNICATIONS EQUIPMENT. 3. Final Completion: Conduct walk-through of the Project work with CLIENT and CONTRACTORS promptly after CONTRACTORS submit notice that entire Project work is complete. Purpose of the walk-through is to determine if work is complete and to visually inspect coating for any damage. CONSULTANT will prepare written list of deficiencies. Time of Beginning and Completion November 3 – November 30, 2016 Compensation CONSULTANT shall be compensated for services included in this Supplemental Agreement on an HOURLY basis for LABOR PLUS REIMBURSABLE EXPENSES in accordance with the prevailing Hourly Fees and Charges Fiscal Year 2016-2017 (Form BC_C 16-17). Except as specifically amended by this Supplemental Agreement, all the terms and conditions of the original Agreement dated shall remain in full force and effect.
