AGENDA ITEM:Consideration of approval of Change Order No. 3 to Absolute Concrete
Construction, Inc. for the NW 10th Street Improvements, W Hickman
Road to NW Douglas Parkway increasing the contract price in the
amount of $28,964.23.
FORMAT:Motion
SYNOPSIS INCLUDING PRO & CON: Change Order No. 3 is for additional grading for the
temporary granular access, insulation over the existing water main, lowering a section of water
main, and pedestrian pushbuttons.
FISCAL IMPACT INCLUDING COST/BENEFIT ANALYSIS: The net result of this
Change Order increases the contract price in the amount of $28,964.23. Of this total change
order amount, Foth will be reimbursing the City $20,653.22 (item CO 3.2) due to a design error.

Dear Ms. Kappos:
Attached please find a detailed Change Order for City Council approval on August 16, 2021.
Change Description
Item 2.4 Excavation, Class 10, Roadway & Borrow
o Increase item for extra grading for temporary access to the Stratford Development.
Item CO 3.1 Insulation of Water Main
o Add item for insulation over shallow main along NW 10th Street.
Item CO 3.2 Time and Material for Lowering Water Main in SE Corner of Sunrise and NW 10th.
o Add item for main lowering to minimum 5 ’ bury.
Item CO 3.3 APS Pedestrian Pushbuttons
o City requested additional pushbuttons in SE corner of Hickman and 10th.
We have reviewed the submitted quotes from Absolute Concrete Construction, Inc. and they appear to be
in order with current industry pricing. If City Staff concurs with this pricing, we recommend the City of
Waukee Increase the contract price in the amount of $28,964.23 to complete the additional work
associated with Change Order #03.
