AGENDA ITEM:Consideration of approval of Change Order No. 2 to Concrete
Technologies, Inc. for the Warrior Lane Improvements Phase 1 Project
increasing the contract amount by $45,618.10.
FORMAT:Consent Agenda
SYNOPSIS INCLUDING PRO & CON: Change Order No. 2 is for a sanitary sewer spot
repair, excavation for gas main relocation, intake lowering,
unincorporated materials, and insulation of existing sanitary sewer force
main.
FISCAL IMPACT INCLUDING COST/BENEFIT ANALYSIS: The net result of this
Change Order increases the contract price in the amount of $45,618.10.

RECOMMENDATION: Approve the change order.

RE: Release of Retainage
Warrior Lane Joint Utility Trench
SH Project Number 4185870
The lien waivers have been received for the above referenced project and the contractor has met the
requirements necessary for the release of the retainage on the project. If city staff concurs, we
recommend the release of the retainage to TD&I Cable Maintenance Inc. of Lakeland, MN in the
amount of $20,656.71
