Technologies, Inc. for the Ute Avenue and Ashworth Road Intersection
Project increasing the contract amount by $119,330.55.
FORMAT:Consent Agenda
SYNOPSIS INCLUDING PRO & CON: Change Order No. 1 is to revise Ashworth Road from
station 118+36 to station 125+30 from a rural section to an urban section. This change is
associated with coordination between the City and the Hamilton Ridge development and includes
revisions to grading, storm sewer, pavement, and erosion control quantities. Substantial
completion date remains unchanged with this change order.
FISCAL IMPACT INCLUDING COST/BENEFIT ANALYSIS: The net result of this
Change Order increases the contract price in the amount of $119,330.55.

RECOMMENDATION: Approve the change order.

UTE AVENUE AND ASHWORTH ROAD INTERSECTION PROJECT
Dear Rudy:
Enclosed is change order 01 for the Ute Avenue and Ashworth Road Intersection project. This
change order includes work associated with revising Ashworth Road from station 118+36 to
station 125+30 from a rural cross section to an urban cross section. This change is associated
with coordination between the City and the Hamilton Ridge development and includes revisions
to grading, storm sewer, pavement, and erosion control quantities.
We recommend approval of change order 01 in the amount of $119,330.55 to the prime
contractor, Concrete Technologies, Inc. of Grimes, Iowa. Substantial completion date remains
unchanged with this change order. Please place this change order on the next available city
council meeting agenda for approval.
