SYNOPSIS INCLUDING PRO & CON: Change Order No. 1 is for the construction of a 12-
inch diameter water main adjustment necessary for the installation of sanitary sewer on the
project.
FISCAL IMPACT INCLUDING COST/BENEFIT ANALYSIS: The net result of this
Change Order increases the contract price in the amount of $12,196.00.

PAY ESTIMATE NO. 2
CHANGE ORDER NO. 1
Enclosed are three copies of executed Pay Estimate No. 2 for work on the Little Walnut Creek North
Trunk Sewer Extension – Phase 1 Completion project, under the contract between the City of Waukee
and J&K Contracting, LLC. dated August 9, 2021. The partial payment estimate is for the period
September 10, 2021 to September 30, 2021.
We have checked the estimate and recommend payment to J&K Contracting, LLC in the amount of
$477,980.39.
Please sign all copies of Pay Estimate No. 2 in the space provided and return one signed copy of the
partial payment estimate to our office. Please forward one signed copy of Pay Estimate No. 2 with
payment to J&K Contracting, LLC.
Pay Estimate No. 2 is for 24” sanitary sewer in place.
Also enclosed are three copies of Change Order No. 1 for the construction of a 12-inch diameter
water main adjustment in the amount of $12,196.00. Change Order No. 1 has been incorporated into
Pay Estimate No. 2.
Please sign all copies of Change Order No. 1 in the space provided and return one signed copy of the
change order to our office. Please forward one signed copy of Change Order No. 1 along with Pay
Estimate No. 2 to J&K Contracting, LLC.
