AGENDA ITEM:Consideration of approval of Change Order No. 1 to Meade, Inc for the
Waukee 10-Inch Project Meredith/NW 10th Street Distribution Main
increasing the contract amount by $11,169.90.
FORMAT:Consent Agenda
SYNOPSIS INCLUDING PRO & CON: Change Order No. 1 is to add an estimated 265 feet
of bore footage of the 10-inch PE gas main along 10th Street crossing Douglas Pkwy. There has
also been one bore extended along 10th Street crossing the Shoeman property, adding an
estimated 365 feet of bore footage of the 10-inch PE gas main. The total bore footage added from
both changes is estimated to be 630 feet.
FISCAL IMPACT INCLUDING COST/BENEFIT ANALYSIS: The net result of this
Change Order increases the contract price in the amount of $11,169.90.

Dear Mr. Royer,
Please disregard the original letter dated 10/01/21 and use this one in its place. All other copies of the
plans are considered void and this set should be used in its place.
At the request of the City of Waukee, we have created a change order to add an estimated 265 feet of
bore footage of the 10-inch PE gas main along 10th St. crossing Douglas Pkwy (approximately station
46+45 to station 50+45). There has also been one bore extended along 10 th St. crossing the Shoeman
property (the bore pit moved from approximately 59+35 to station 63+00, to be combined with an
existing bore pit), adding an estimated 365 feet of bore footage of the 10-inch PE gas main. The total
bore footage added from both changes is estimated to be 630 feet.
The tee, valve, stub, and cap, approximately located at station 103+75 has been moved to
approximately station 53+10. The two 10-inch valves located approximately at stations 103+55 &
104+80 have been moved to approximately stations 103+25 and 100+70.
The City will provide the materials. USDI has reviewed the attached Change Order dated October 26,
2021 and recommend it be approved. The total estimated additional cost for this work is $11,169.90
plus materials.
