RESOLUTION NO.99-64
A RESOLUTION APPROVING CHANGE ORDER NO.2 TO THE CONTRACT
WITH RICE LAKE CONTRACTING CORP.FOR THE WASTEWATER
TREATMENT FACILITY IMPROVEMENTS INCREASING THE CONTRACT
IN THE AMOUNT OF $30,484.00
WHEREAS,it has been determined that the a change order is both necessary and
prudent for the following modifications:
1.Addition of coarse bubble diffusers in the biosolids storage tank.
2.Changing the aeration system from 5 -30 HP blowers to 3 -40 blowers and the
elimination of one diffuser assembly per aeration chain.
3.Addition of rough-in plumbing by the contractor for the installation of a sink in the
process room of the headworks building.
4.Addition of a clay plug on Reed Beds subnatant line.
5.Increase the size ofMH-l from 48"diameter to 60"diameter.
6.Moving of tap location with the installation of a II-v"0 bend to bring water main in
back line.
7.Extra work for repairing a 24"clay tile line which was not at the location as shown on
plans.
8.Addition of two II-v"0 bends on water line to go around 24"clay tile.
9.Addition of a structure to accommodate a conflict between existing piping to the
lagoons and the new 24"RW.
10.Installation of new 24"DIP between stromsewer release structure and MH-l.
11.Addition of three 2"pvc true union ball valves on the sampling lines at the sampler in
the Control Room.
NOW THEREFORE,BE IT RESOLVED BY THE CITY COUNCIL OF THE
CITY OF WAUKEE,IOWA,that Change Order No.2 increasing the amount of the
contract with Rice Lake Contracting Corp.in the amount of $30,484.00 for the
Wastewater Treatment Facility Improvements is hereby approved;)
BE IT FURTHER RESOLVED,that the Mayor and City Clerk be and they are hereby
authorized and directed to execute said Change Order No.2.
PASSED AND APPROVED this 21st day of June,1999.
